There was a comic mini-series about him but I can't recall how much detail it went into his origin. It most likely addresses some of your questions if you ever take a look at it (very thin graphic novel).

I don't think he's done any covers. He worked on a lot of the guides: Guide to Starships, KOTOR campaign guide, Threats to the Galaxy, and TFU campaign guide. I don't have any of these books so I'm not sure how many illustrations he did in each book.

I think Katie Cook is a fantastic artist. I don't know if we're counting the TCW web-comics as EU, but she also does some of the Topps SW Galaxy sets. I just love the cutsey look. To me it doesn't simplify the Saga in the slightest.

Also, I really like Cynthia Cummens. Her art is very feminine. She's does some 'You Can Draw Star Wars' as well as some of the Topps sets as well.
