I'm pretty sure I've read that he slowly improved upon the design over the course of the years. That he made a tweak here, and adjustment there.
But remember that the suit was damaged pretty severely in The Force Unleashed. He may have had to replace it altogether, but it could have been repaired.
Also, Shadows of the Empire says that he could, and did, regularly remove the suit in his meditation pod aboard Executor. That implies to me that he could, if he wanted, alternate between several suits as needed (e.g. Suit A needs maintenance, so I'll wear Suit B this week).
That said, he may have simply always tweaked and adjusted the same suit for all those years. But I guarantee that, due to damage done by Rebels/Jedi and the overall adjustments Vader made to it, the suit was not precisely the same as the one he was given in ROTS.
